Abstract

|
연구목적 : 상급종합병원과 종합병원에서 간호·간병통합서비스를 제공하는 데 필요한 간호인력 배치수준과 간호전달체계를 개발하였다.
연구방법 : 간호·간병통합서비스를 제공하고 있지 않은 5개 병원의 10개 병동에서 시간-동작 관찰조사를 실시하여 간호인력(간호사, 간호보조인력)과 환자 가족 및 간병인이 제공한 간호행위와 시간을 측정하였다. 관찰조사 병동을 포함한 80개 병동에서 간호관리자 설문조사를 실시하여 간호인력 배치수준과 환자 간호요구도(환자분류체계 점수)를 파악하였다.
연구결과 : 관찰조사 병동과 설문조사 병동의 근무조별 간호사 1인당 환자수는 각각 1:10.1과 1:11.1이었다. 간호인력과 환자 가족/간병인이 제공한 환자 1인당 일평균 직접간호시간은 각각 51분과 130분이었다. 환자분류체계 점수가 높을수록 직접간호시간이 증가하는 상관관계를 보였다. 환자분류체계 점수에 근거했을 때 간호·간병통합서비스 제공을 위한 상급종합병원 간호인력 배치수준은 1:3.9에서 1:6.1이었고, 종합병원 배치수준은 1:4.4에서 1:6.0이었다. 현행 간호전달체계로서 기능적 간호를 적용하고 있는 병동은 38.5%이었다.
결론: 안전한 간호·간병통합서비스 제공을 위해 적정 간호인력을 배치하고 효율적인 간호전달체계를 개발하는 것이 필요하다.

Purpose: To develop staffing levels for nursing personnel (registered nurses and nursing assistants) to provide inpatients with integrated nursing care that includes, in addition to professional nursing care, personal care previously provided by patients' families or private caregivers.
Methods: A time & motion study was conducted to observe nursing care activities and the time spent by nursing personnel, families, and private caregivers in 10 medical-surgical units. The Korean Patient Classification System-1 (KPCS-1) was used for the nurse manager survey conducted to measure staffing levels and patient needs for nursing care.
Results: Current nurse to patient ratios from the time-motion study and the survey study were 1:10 and 1:11, respectively. Time spent in direct patient care by nursing personnel and family/private caregivers was 51 and 130 minutes per day, respectively. Direct nursing care hours correlated with KPCS-1 scores. Nursing personnel to patient ratio required to provide integrated inpatient care ranged from 1:3.9 to 1:6.1 in tertiary hospitals and from 1:4.4 to 1:6.0 in general hospitals. The functional nursing care delivery system had been implemented in 38.5% of the nursing units.
Conclusion: Findings indicate that appropriate nurse staffing and efficient nursing care delivery systems are required to provide integrated inpatient nursing care.
